Purpose Sepsis survivors have a higher risk of rehospitalisation and of long-term mortality. We assessed the rate, diagnosis, and independent predictors for rehospitalisation in adult sepsis survivors. Methods We searched for non-randomized studies and randomized clinical trials in MEDLINE, Cochrane Library, Web of Science, and EMBASE (OVID interface, 1992-October 2019). The search strategy used controlled vocabulary terms and text words for sepsis and hospital readmission, limited to humans, and English language. Two authors independently selected studies and extracted data using predefined criteria and data extraction forms. Results The literature search identified 12,544 records. Among 56 studies (36 full and 20 conference abstracts) that met our inclusion criteria, all were non-randomised studies. Studies most often report 30-day rehospitalisation rate (mean 21.4%, 95% confidence interval [CI] 17.6-25.4%; N = 36 studies reporting 6,729,617 patients). The mean (95%CI) rehospitalisation rates increased from 9.3% (8.3-10.3%) by 7 days to 39.0% (22.0-59.4%) by 365 days. Infection was the most common rehospitalisation diagnosis. Risk factors that increased the rehospitalisation risk in sepsis survivors were generic characteristics such as older age, male, comorbidities, non-elective admissions, hospitalisation prior to index sepsis admission, and sepsis characteristics such as infection and illness severity, with hospital characteristics showing inconsistent associations. The overall certainty of evidence was moderate for rehospitalisation rates and low for risk factors. Conclusions Rehospitalisation events are common in sepsis survivors, with one in five rehospitalisation events occurring within 30 days of hospital discharge following an index sepsis admission. The generic and sepsis-specific characteristics at index sepsis admission are commonly reported risk factors for rehospitalisation. Registration PROSPERO CRD 42016039257, registered on 14-06-2016.

Reactions of 2-[1-(3,5-dimethylpyrazol-1-yl)ethyl]pyridine (L1) and 2-[1-(3,5-diphenylpyrazol-1-yl)ethyl]pyridine (L2) with the [Pd (COD)Cl-2] or [Pd (COD)MeCl] produced palladium (II) complexes [Pd(L1)ClMe] (1), [Pd(L1)Cl-2] (C2), [Pd(L2)ClMe] (3), and [Pd(L2)Cl-2] (4) in quantitative yields. Solid state structures of complexes 1, 3 and 4 established the formation of mononuclear compounds, containing one bidentate ligand unit per metal atom, to give square planar complexes. All the other spectroscopic characterization data and elemental analyses were consistent with the observed structures. All the palladium (II) complexes 1-4 gave active catalysts in the methoxycarbonylation of 1-octenes. The catalysts demonstrated 100% chemoselectivities towards esters and favored the formation of linear isomers. Reaction conditions such as the type of phosphine derivative, acid promoter, solvent system, time, pressure and temperature have been investigated and shown to affect both the catalytic activity and regio-selectivity of the catalysts. Solid-angle modelling established the comparable steric contributions from the ligands, consistent with the similar regioselectivities of the resultant catalysts.

Ampelocalamus scandens is native to Guizhou Province, China, and grows at an altitude of 260-320 m. It can be used as a raw material for weaving and papermaking. In the current study, the complete chloroplast (cp) genome of A. scandens was sequenced and is reported for the first time. The complete cp sequence was 139,504 bp, include large single-copy (LSC), small single-copy (SSC), and a pair of invert repeats (IR) region of 83,103 bp, 12,813 bp, and 21,793 bp, respectively. Besides, the plastid genome comprised a total of 132 genes, including protein-coding, tRNA, and rRNA genes as 85, 39, and 8 genes, respectively. Phylogenetic analysis based on 28 cp genomes reveals that A. scandens is closely associated with Ampelocalamus melicoideus in Arundinarodae.

An enantioselective Mannich reaction between 3-fluorooxindoles and pyrazolinone ketimines has been developed for the construction of amino-pyrazolone-oxindoles containing stereogenic C-F units. Based on this new protocol that allows for the generation of two adjacent tetrasubstituted stereocenters, a variety of structurally diverse fluorinated amino-pyrazolone-oxindoles were obtained in good to excellent yields with excellent diastereoselectivities and enantioselectivities (up to 98% yield, >20 : 1 dr and >99% ee). What’s more, good yield and high stereoselectivities were obtained in the gram-scale reaction.

The Pd(ii)-catalyzed activation of Csp(2)-H bond and double alkyne annulation which proceeds via allylic isomerization is reported for the first time. This reaction of antipyrines with alkynes provides an efficient synthetic route for the biologically important spiro-cyclopentadiene pyrazolones. In the presence of Lawesson’s reagent, this Pd(ii)-catalyzed annulation reaction affords another spiro-cyclopentadiene pyrazolone which displays very good fluorescence properties.

Purpose: Regorafenib is an oral multikinase inhibitor approved for the therapy of previously treated metastatic colorectal carcinoma (mCRC). The aim of the present study was to analyze the outcomes of treatment with regorafenib in real-world clinical practice based on data from a national registry. Methods: The CORECT registry, the Czech non-interventional database of patients with mCRC treated with targeted agents, searched for patients with metastatic CRC treated with regorafenib. In total, 555 evaluable patients were identified. Results: The median age at diagnosis was 61.7 years. All patients had disease progression on or after previous systemic treatment. Most patients were treated with an initial dose of 160 mg daily (n = 463; 83.6%). The median duration of treatment was 2.7 months (range 0.023.4 months). By the data cut-off date, 472 patients (85%) had completed treatment with regorafenib and were evaluable for treatment response evaluation. Partial response was reported in 13 patients (2.8%) and disease stabilization in 130 patients (27.5%). Median progression-free survival (PFS) and overall survival (OS) were 3.5 months (95% confidence interval [CI] 3.2-3.7 months) and 9.3 months (95% CI 8.3-10.3 months), respectively. The 6-month OS rate was 67.7% (95% CI 63.4-72.1%). Multivariable analysis showed that female gender, longer interval from diagnosis of metastatic disease, MO stage at diagnosis, and Eastern Cooperative Oncology Group performance status (ECOG PS) 0 were associated with longer PFS, while higher body-mass index (BMI), longer interval from diagnosis of metastatic disease, and ECOG PS of 0 were associated with longer OS. Conclusion: OS of patients treated with regorafenib in the real-world clinical practice in this cohort exceeded that reported in randomized trials. Regorafenib is a safe and active treatment option for a subgroup of patients with mCRC who are progressing after other systemic therapies and maintain good performance status.

Monomeric zinc ferrocene carboxylate [Zn(FcCOO)(3,5-dmp)(2)Cl] (1) has been synthesized from a reaction between the zinc chloride, sodium salt of ferrocene carboxylic acid (FcCOONa) and 3,5-dimethylpyrazole (3,5-dmp) at room temperature. Compound 1 was further characterized by elemental analysis, H-1 NMR, FT-IR, absorption, emission spectroscopic techniques, and molecular structure was confirmed by single-crystal X-ray diffraction studies. The molecular structure of compound 1 crystallized in triclinic system with P (1) over bar space group and the central Zn2+ ion is in a distorted tetrahedral geometry with -FeN2OCl coordination environment. Compound 1 was further stabilized with the aid of inter and intramolecular hydrogen bonding and pi-pi interaction between cyclopentadienyl ring of ferrocene and aromatic heterocyclic five membered 3,5-dimethylpyrazole. Compound 1 exhibits broad emission band, indicating ligandto-metal charger transfer (LMCT) or metal-to-ligand charger transfer (MLCT) nature. The electrochemical property of compound 1 revealed that one-electron anodic and cathodic peaks corresponding to the redox responsible of ferrocene/ferrocenium (FeII <-> Fe-III) moiety. Among the different strains of bacteria, the antibacterial activity of compound 1 showed comparatively a significant activity against gram negative bacterium Proteus vulgaris. (C) 2020 Elsevier B.V. Two unexpected one-dimensional coordination polymers, [Cu(PT)(H2O)Cl](n) 1 and [Cu-2(BPT)(ClO4)(3)(H2O)(4)](n)center dot 2nH(2)O 2, of symmetrical triazine-based ligands were synthesized by Cu(II)-mediated hydrolysis of the 2,4-bis(3,5-dimethyl-1H-pyrazol-1-yl)-6-methoxy-1,3,5-triazine (MBPT) pincer ligand. The reaction of Cu(ClO4)(2)center dot 6H(2)O with MBPT proceeded via hydrolysis of the methoxy group to produce the dicompartmental 4,6-bis(3,5-dimethyl-1H-pyrazol-1-yl)-1,3,5-triazin-2(1H)-one ligand (HBPT) that then undergoes in situ complexation with Cu(II) to afford 2. In case of CuCl2, the reaction proceeds further with C-N cleavage of one pyrazolyl unit leading to the formation of 6-(3,5-dimethyl-1H-pyrazol-1-yl)-1,3,5-triazine-2,4(1H,3H)-dione ligand (HPT) that also undergoes in situ complexation with Cu(II) affording 1. The role of Cu(II) is to increase the Lewis acid reactivity of the water molecule where similar hydrolytic reactions for MBPT were observed in acidic medium in presence of an aqueous HCl (1:1 v/v) solution. The molecular and supramolecular structures of complexes 1 and 2 were investigated using X-ray diffraction of single crystal, Hirshfeld analysis, and density functional theory calculations. The Cl center dot center dot center dot H (11.7%) and O center dot center dot center dot H (24.7%) contacts are the most important in 1, whereas the molecular packing of 2 is controlled mainly by the O center dot center dot center dot H (58.7%) hydrogen bonds. Complex 2 showed better activity against Escherichia coli, Bacillus subtilis, and Candida albicans compared with the standard antibiotics amoxicillin, tetracycline, and ampicillin. In general, complexes 1 and 2 showed good antimicrobial activity than these antibiotics and have the advantage to be used as both antibacterial and antifungal agents.

Aerobic organisms must rely on abundant intracellular thiols to reductively protect various vital functional units, especially ubiquitous zinc(II) thiolate sites of proteins, from deleterious oxidations resulting from oxidizing environments. Disclosed here is the first well-defined model study for reactions between zinc(II) thiolate complexes and copper(II) complexes. Among all the studied ligands of copper(II), diethyldithiocarbamate (DTC) displays a unique redox-tuning ability that enables copper(II) to resist the reduction by thiols while retaining its ability to oxidize zinc(II) thiolates to form disulfides. This work proves for the first time that it is possible to develop oxidants to discriminate between thiols and zinc(II) thiolates, alluding to a new chemical principle for how oxidants, especially universal anticancer Cu(DTC)(2), might circumvent the intracellular reductive defense around certain zinc(II) thiolate sites of proteins to kill malignant cells.
