Purpose: Regorafenib is an oral multikinase inhibitor approved for the therapy of previously treated metastatic colorectal carcinoma (mCRC). The aim of the present study was to analyze the outcomes of treatment with regorafenib in real-world clinical practice based on data from a national registry. Methods: The CORECT registry, the Czech non-interventional database of patients with mCRC treated with targeted agents, searched for patients with metastatic CRC treated with regorafenib. In total, 555 evaluable patients were identified. Results: The median age at diagnosis was 61.7 years. All patients had disease progression on or after previous systemic treatment. Most patients were treated with an initial dose of 160 mg daily (n = 463; 83.6%). The median duration of treatment was 2.7 months (range 0.023.4 months). By the data cut-off date, 472 patients (85%) had completed treatment with regorafenib and were evaluable for treatment response evaluation. Partial response was reported in 13 patients (2.8%) and disease stabilization in 130 patients (27.5%). Median progression-free survival (PFS) and overall survival (OS) were 3.5 months (95% confidence interval [CI] 3.2-3.7 months) and 9.3 months (95% CI 8.3-10.3 months), respectively. The 6-month OS rate was 67.7% (95% CI 63.4-72.1%). Multivariable analysis showed that female gender, longer interval from diagnosis of metastatic disease, MO stage at diagnosis, and Eastern Cooperative Oncology Group performance status (ECOG PS) 0 were associated with longer PFS, while higher body-mass index (BMI), longer interval from diagnosis of metastatic disease, and ECOG PS of 0 were associated with longer OS. Conclusion: OS of patients treated with regorafenib in the real-world clinical practice in this cohort exceeded that reported in randomized trials. Regorafenib is a safe and active treatment option for a subgroup of patients with mCRC who are progressing after other systemic therapies and maintain good performance status.

The reaction of 3,4,4-trichloro-1,1-bis(3,5-dimethyl-1H-pyrazol-1-yl)-2-nitrobuta-1,3-diene with 2-aminopyridine gave a polysubstituted pyrido[1,2-a]pyrimidine derivative with an aminopyridine residue in the 2-position, whereas heterocyclization of 3,4,4-trichloro-1,1-bis(1H-benzotriazol-1-yl)-2-nitrobuta-1,3-diene with substituted 2-aminopyridines afforded 2-(1H-berrzotriazol-1-yl)pyrido[1,2-a]pyrimidines. 2-(Morpholin-4-yl), 2-amino, and 2-(2-hydroxyethylamino) derivatives of polysubstituted pyrido[1,2-a]pyrimidines were synthesized, and those containing a 2-hydroxyethylamino group were converted to the corresponding 4,5-dichloro-1,2-thiazole-1-carboxylates. Specific features of heterocyclizations of benzotriazolyl and dimethylpyrazolyl derivatives of trichloronitro-1,3-butadiene to pyrimidine and pyrazole systems were revealed.

Electrocatalytic hydrogenation is a particularly attractive approach for converting the most unstable compounds in biogenic feedstocks at ambient conditions without external H-2. Here, we synthesized a variety of carbon-supported transition metal catalysts and characterized their activity for the electrocatalytic hydrogenation of a series of model compounds and pyrolysis bio-oil. Carbonyl compounds, especially aromatic aldehydes, such as furfural and benzaldehyde, are particularly inclined to hydrogenation driven by an applied current. This was verified with pure solutions of the model compounds and with pyrolysis bio-oil, where we achieved stable and steady continuous operation on Pd. When optimal catalyst composition was chosen, the conversion of benzaldehyde shifted from alcohol production (e.g., on Pd and Cu) to dimerization (e.g., on Co, Ni, and Zn). Pd and Cu were shown to offer the best compromise between reaction rates and efficiency although, in general, base metals offer similar conversions but better efficiencies than noble metals. Thus, the present work offers foundational results and guidelines for choosing the optimal metal catalyst and the applied potential for processing organic feedstocks as a function of its composition.

Background: The high prevalence of physician burnout, particularly in emergency medicine, has garnered national attention in recent years. Objective means of measuring stress while at work can facilitate research into stress reduction interventions, and wearable photoplethysmography (PPG) technology has been proposed as a potential solution. However, the use of low-burden wearable biosensors to study training and clinical practice among emergency physicians (EP) remains untested. Objective: This pilot study aimed to (1) determine the feasibility of recording on-shift photoplethysmographic data from EP, (2) assess the quality of these data, and (3) calculate standard pulse rate variability (PRV) metrics from the acquired dataset and examine patterns in these variables over the course of an academic year. Methods: A total of 21 EP wore PPG biosensors on their wrists during clinical work in the emergency department during a 9-hour shift. Recordings were collected during the first quarter of the academic year, then again during the fourth quarter of the same year for comparison. The overall rate of usable data collection per time was computed. Standard pulse rate (PR) and PRV metrics from these two time points were calculated and entered into Student t tests. Results: More than 400 hours of data were entered into these analyses. Interpretable data were captured during 8.54% of the total recording time overall. In the fourth quarter of the academic year compared with the first quarter, there was no significant difference in median PR (75.8 vs 76.8; P=.57), mean R-R interval (0.81 vs 0.80; P=.32), SD of R-R interval (0.11 vs 0.11; P=.93), root mean square of successive difference of R-R interval (0.81 vs 0.80; P=.96), low-frequency power (3.5×103 vs 3.4×103; P=.79), high-frequency power (8.5×103 vs 8.3×103; P=.91), or low-frequency to high-frequency ratio (0.42 vs 0.41; P=.43), respectively. Power estimates for each of these tests exceeded .90. A secondary analysis of the resident-only subgroup similarly showed no significant differences over time, despite power estimates greater than .80. Conclusions: Although the use of PPG biosensors to record real-time physiological data from EP while providing clinical care seems operationally feasible, this study fails to support the notion that such an approach can efficiently provide reliable estimates of metrics of interest. Four new derivatives of the versatile polyfunctional ligands 3-(pyrazol-1-yl)propanamide (1, = PPA) and 3-(3,5-dimethylpyrazol-1-yl)-propanamide (2, = Me(2)PPA) have been prepared, in which one of the amide hydrogens on 1 and 2 is formally replaced by alkyl residues. X-Ray diffraction studies revealed that, in contrast to 1 and 2, the N-isopropyl substituted derivatives 3 and 4 form supramolecular hydrogen-bonded chains rather than two-dimensional arrays, and the N-(2-methyl-4-oxypentan-2-yl) (=MOP) substituted compounds 5 and 6 exist as cyclic dimers in the solid state. Reactions of PdCl2(COD) (COD = 1,5-cyclooctadiene) with 2 equiv. of the corresponding ligands 1-6 in all cases afforded trans-PdCl2(L)(2) complexes (7-12). X-ray crystal structure determinations of 8, and 10-12 revealed kappa N-monodentate coordination of the PPA-type ligands. (C) 2019 Elsevier Ltd. All rights reserved.

A practical means to quantify the response of the rates of net N mineralization and denitrification over a wide range of soil water contents is generally lacking. This study examined the potential to use a nitrification inhibitor (NI) assay system to simultaneously estimate the rates of net N mineralization and denitrification, and applied the NI assay to assess the effect of water content on net N mineralization and denitrification rates in two soils with contrasting soil texture. The compound 3,5-dimethylpyrazole (DMP) applied at a rate of 200 mg kg(-1) was found to provide essentially complete inhibition of nitrification over the duration of the soil incubation for two soils with contrasting soil texture (clay loam vs. sandy loam) and over a range of soil water contents (35%, 55%, and 85% water-filled pore space). This allowed net N mineralization to be estimated as the accumulation of soil ammonium (NHthorn 4) and of denitrification as the disappearance of added nitrate (NO3-). Addition of DMP resulted in a small increase in soil respiration rate but did not appear to influence the rate of net soil N mineralization. The NI assay provides a practical means to quantify the rates of net N mineralization and denitrification simultaneously over a wide range of soil water contents. The assay can be readily scaled up to routinely test multiple soils in an efficient manner, has limited material costs, and is also relatively simple to perform.
