In the process of research and development of new drugs, in silico analyzes are widely used. They address the pharmacokinetics of the molecules in study and can predict the binding mode and affinity, using a docking software. This approach can optimize the development of new drugs, reducing costs, time, and resources. In this study, a library of 300 pyrazole-chalcone derivatives were designed, the in silico ADMET (absorption, distribution, metabolism, excretion, and toxicity) properties were evaluated, and a structure-based virtual screening was performed using AutoDock Vina. The docking results exhibited that the derivatives binding mode at the COX-2 active site is similar to celecoxib, the reference drug, and presented similar binding energy. Six compounds were synthetized and tested for in vitro inhibition of the COX-1 and COX-2 isoenzymes and the selectivity index (SI) was calculated. The compound 2a11 showed the best activity for COX-2 (IC50COX-2=0.73M) whereas the control, celecoxib, resulted IC50COX-2=0.88M. All the other compounds synthetized presented better potency for COX-2 inhibition than the control. Compound 2a23 exhibited the higher SI, of 280.17 (IC50COX-1=210.13M/ IC50COX-2=0.75M), while celecoxib was 246.88 (IC50COX-1=217.26M/ IC50COX-2=0.88M). These results corroborate with a possible anti-inflammatory activity and COX-2 selectivity of the new compounds synthetized.

Background: The high prevalence of physician burnout, particularly in emergency medicine, has garnered national attention in recent years. Objective means of measuring stress while at work can facilitate research into stress reduction interventions, and wearable photoplethysmography (PPG) technology has been proposed as a potential solution. However, the use of low-burden wearable biosensors to study training and clinical practice among emergency physicians (EP) remains untested. Objective: This pilot study aimed to (1) determine the feasibility of recording on-shift photoplethysmographic data from EP, (2) assess the quality of these data, and (3) calculate standard pulse rate variability (PRV) metrics from the acquired dataset and examine patterns in these variables over the course of an academic year. Methods: A total of 21 EP wore PPG biosensors on their wrists during clinical work in the emergency department during a 9-hour shift. Recordings were collected during the first quarter of the academic year, then again during the fourth quarter of the same year for comparison. The overall rate of usable data collection per time was computed. Standard pulse rate (PR) and PRV metrics from these two time points were calculated and entered into Student t tests. Results: More than 400 hours of data were entered into these analyses. Interpretable data were captured during 8.54% of the total recording time overall. In the fourth quarter of the academic year compared with the first quarter, there was no significant difference in median PR (75.8 vs 76.8; P=.57), mean R-R interval (0.81 vs 0.80; P=.32), SD of R-R interval (0.11 vs 0.11; P=.93), root mean square of successive difference of R-R interval (0.81 vs 0.80; P=.96), low-frequency power (3.5×103 vs 3.4×103; P=.79), high-frequency power (8.5×103 vs 8.3×103; P=.91), or low-frequency to high-frequency ratio (0.42 vs 0.41; P=.43), respectively. Power estimates for each of these tests exceeded .90. A secondary analysis of the resident-only subgroup similarly showed no significant differences over time, despite power estimates greater than .80. Conclusions: Although the use of PPG biosensors to record real-time physiological data from EP while providing clinical care seems operationally feasible, this study fails to support the notion that such an approach can efficiently provide reliable estimates of metrics of interest. No significant differences in PR or PRV metrics were found at the end of the year compared with the beginning. Although these methods may offer useful applications to other domains, it may currently have limited utility in the contexts of physician training and wellness.

This study provides a phenomenological method to describe the exponential growth, saturation, and decay of coronavirus disease 2019 (COVID-19) deaths and infections via a Monte Carlo approach. The calculations connect Gompertz-type trial distributions of infected people per day with the distribution of deaths adopting two gamma distributions to account for the elapsed time that encompass the incubation and symptom onset to death periods. The analyses include death data from the USA, Brazil, Mexico, the United Kingdom (UK), India, and Russia, which comprise the four countries with the highest number of deaths and the four countries with the highest number of confirmed cases, as of August 07, 2020, according to theWorld Health Organization webpage. The Gompertz functions were fitted to the data of weekly averaged confirmed deaths per day by mapping the chi(2) values. The uncertainties, variances, and covariances of the model parameters were calculated by propagation, taking into account the standard errors of the data for each epidemiological week. The fitted functions for the average deaths per day for the USA and India have an upward trend, with the former having a higher growth rate and quite huge uncertainties. For Mexico, the UK, and Russia, the fits are consistent with a downward-sloping pattern. For Brazil we found a subtle trend down but with significant uncertainties. The USA, UK, and India data showed first peaks with higher growth rates compared with the second ones (4.2, 2.2, and 3.5 times higher, respectively), demonstrating the benefits of nonpharmacological interventions of sanitary measures and social distance flattening the secondary peaks of the pandemic. For the case of the USA, however, a third peak seems quite plausible, most likely related with the recent relaxation policies. Brazil’s data are satisfactorily described by two highly overlapped Gompertz functions with similar growth rates, suggesting a two-step process for the pandemic spreading. For the cases of Mexico and Russia single peaks with smoother slopes fitted the data satisfactorily. The 95% confidence intervals for the total number of deaths (x10(3)) predicted by the model for August 31, 2020, are 160 to 220, 110 to 130, 59 to 62, 41.3 to 41.4, 54 to 63, and 16.0 to 16.7 for the USA, Brazil, Mexico, the UK, India, and Russia, respectively. Our estimates for the point prevalences of infections are compared with some preliminary data from serological studies and/or model calculations focused on the USA, Brazil, and UK scenarios. The point prevalences and 95% confidence intervals for August 1, 2020, were estimated to be 5.7 (3.9-7.5)%, 8.9 (7.4-10.3)%, 9.3 (8.3-10.3)%, 5.7 (4.5-6.9)%, 0.9 (0.8-1.0%), and 1.2 (1.0-1.3)% for the USA, Brazil, Mexico, the UK, India, and Russia, respectively. Purpose Sepsis survivors have a higher risk of rehospitalisation and of long-term mortality. We assessed the rate, diagnosis, and independent predictors for rehospitalisation in adult sepsis survivors. Methods We searched for non-randomized studies and randomized clinical trials in MEDLINE, Cochrane Library, Web of Science, and EMBASE (OVID interface, 1992-October 2019). The search strategy used controlled vocabulary terms and text words for sepsis and hospital readmission, limited to humans, and English language. Two authors independently selected studies and extracted data using predefined criteria and data extraction forms. Results The literature search identified 12,544 records. Among 56 studies (36 full and 20 conference abstracts) that met our inclusion criteria, all were non-randomised studies. Studies most often report 30-day rehospitalisation rate (mean 21.4%, 95% confidence interval [CI] 17.6-25.4%; N = 36 studies reporting 6,729,617 patients). The mean (95%CI) rehospitalisation rates increased from 9.3% (8.3-10.3%) by 7 days to 39.0% (22.0-59.4%) by 365 days. Infection was the most common rehospitalisation diagnosis. Risk factors that increased the rehospitalisation risk in sepsis survivors were generic characteristics such as older age, male, comorbidities, non-elective admissions, hospitalisation prior to index sepsis admission, and sepsis characteristics such as infection and illness severity, with hospital characteristics showing inconsistent associations. The overall certainty of evidence was moderate for rehospitalisation rates and low for risk factors. Conclusions Rehospitalisation events are common in sepsis survivors, with one in five rehospitalisation events occurring within 30 days of hospital discharge following an index sepsis admission. The generic and sepsis-specific characteristics at index sepsis admission are commonly reported risk factors for rehospitalisation. Registration PROSPERO CRD 42016039257, registered on 14-06-2016.

In this letter, an efficient one-pot asymmetric sequential reaction is achieved by organo/transition metal relay catalysis in constructing two consecutive C-C bonds, which involves enantioselective amino squaramide catalytic Mannich-type addition of pyrazolones to isatin-derived ketimines and a subsequent palladium catalyzed diastereoselective allylic alkylation of pyrazolones with allylic acetates. An array of novel pyrazolone-aminooxindole-propylene structural motifs are obtained in a high level of yield and with excellent enantio- and diastereoselectivity (up to 95% yield, >20:1 dr, >99% ee). This methodology features the formation of vicinal quarternary carbon-stereocenters, and the second all-carbon tetrasubstituted stereogenic center is induced by joint action of the achiral palladium catalysis and the chiral environment generated from the Mannich step. Moreover, the usefulness of this methodology is highlighted by converting the allylic product into the carbonyl compound.

Two new coordination complexes of Cu(ii) viz.; [Cu(bpy)(2)(NO3)]NO3 center dot 5H(2)O (1) and [Cu(phen)(Hdmpz)Cl-2]center dot H2O (2) (bpy = 2,2 ‘-bipyridine, phen = 1,10-phenanthroline, Hdmpz = 3,5-dimethyl pyrazole) have been synthesized and characterized by single crystal X-ray diffraction, electronic spectroscopy, EPR, FT-IR spectroscopy and TGA. The nitrate anion and the water molecules in the lattice of 1 are involved in the formation of unconventional infinite supramolecular crown-like nitrate-water clusters. The unconventional enclathration of guest cationic complex moieties of 1 within the supramolecular nitrate-water crown hosts provides stability to the crystal structure. Crystal structure analysis of 2 reveals the presence of cooperative ternary pi-stacked assemblies with enclathration of single guest water molecules in hexameric Cu(II) supramolecular host cavities. Theoretical calculations have been performed to analyse the non-covalent interactions in the compounds; focusing on the unusual H-bonding network/anion-pi cooperative assemblies in 1 and two different pi-stacked cooperative assemblies in 2. DFT studies reveal that the distribution and orientation of the H-bonds in the cooperative H-bonding network in 1 is the most favoured. The high interaction energies of (pi-pi)(1) and (pi-pi)(2) in pi-stacked ternary assemblies of 2 can be attributed to the participation of the entire aromatic surfaces with antiparallel orientations and the cooperative C-H center dot center dot center dot Cl interactions respectively. The in vitro antiproliferative activities of the compounds considering cell cytotoxicity, apoptosis and DNA fragmentation assays reveal significant concentration dependent cytotoxicity in DL cancer cells with nominal effects in normal PBMC cells. The molecular docking and pharmacophore studies established the structure activity relationship of the compounds.

This study provides a phenomenological method to describe the exponential growth, saturation, and decay of coronavirus disease 2019 (COVID-19) deaths and infections via a Monte Carlo approach. The calculations connect Gompertz-type trial distributions of infected people per day with the distribution of deaths adopting two gamma distributions to account for the elapsed time that encompass the incubation and symptom onset to death periods. The analyses include death data from the USA, Brazil, Mexico, the United Kingdom (UK), India, and Russia, which comprise the four countries with the highest number of deaths and the four countries with the highest number of confirmed cases, as of August 07, 2020, according to theWorld Health Organization webpage. The Gompertz functions were fitted to the data of weekly averaged confirmed deaths per day by mapping the chi(2) values. The uncertainties, variances, and covariances of the model parameters were calculated by propagation, taking into account the standard errors of the data for each epidemiological week. The fitted functions for the average deaths per day for the USA and India have an upward trend, with the former having a higher growth rate and quite huge uncertainties. For Mexico, the UK, and Russia, the fits are consistent with a downward-sloping pattern. For Brazil we found a subtle trend down but with significant uncertainties. The USA, UK, and India data showed first peaks with higher growth rates compared with the second ones (4.2, 2.2, and 3.5 times higher, respectively), demonstrating the benefits of nonpharmacological interventions of sanitary measures and social distance flattening the secondary peaks of the pandemic. For the case of the USA, however, a third peak seems quite plausible, most likely related with the recent relaxation policies. Brazil’s data are satisfactorily described by two highly overlapped Gompertz functions with similar growth rates, suggesting a two-step process for the pandemic spreading. For the cases of Mexico and Russia single peaks with smoother slopes fitted the data satisfactorily. The 95% confidence intervals for the total number of deaths (x10(3)) predicted by the model for August 31, 2020, are 160 to 220, 110 to 130, 59 to 62, 41.3 to 41.4, 54 to 63, and 16.0 to 16.7 for the USA, Brazil, Mexico, the UK, India, and Russia, respectively. Our estimates for the point prevalences of infections are compared with some preliminary data from serological studies and/or model calculations focused on the USA, Brazil, and UK scenarios. The point prevalences and 95% confidence intervals for August 1, 2020, were estimated to be 5.7 (3.9-7.5)%, 8.9 (7.4-10.3)%, 9.3 (8.3-10.3)%, 5.7 (4.5-6.9)%, 0.9 (0.8-1.0%), and 1.2 (1.0-1.3)% for the USA, Brazil, Mexico, the UK, India, and Russia, respectively. The method represents an effective few-parameter framework to estimate the line shape of the infection curves and the uncertainties of the relevant parameters based on the actual death data of a pandemic.
