The current work investigates the performance of a real-time scan conversion algorithm for generating a 2-D ultrasound image from a laterally scanned single-element ultrasound transducer, which has applications in point-of-care devices such as for skin imaging. The algorithm employs a fixed calibration curve to update a predefined image grid in real time. Simulations showed that the calibration curve (with a maximum of 1) is robust to changes in scatterer concentration (8.3 x 10(-3) mean absolute error), signal to noise ratio (1.0 x 10(-3) mean absolute error for -5 dB SNR), and can be accurately predicted from a small number (31) of point scatterers (6.9 x 10(-3) mean absolute error). Good agreement was also found between the calibration curves obtained from simulated and experimental data (1.19 x 10(-2) mean absolute error). The scan conversion algorithm was validated by evaluation of the position estimation errors on both simulations and experiments. Clinical images of skin lesions (N = 20) demonstrate the feasibility of the algorithm for real, non-homogeneous tissue. Use of a fixed calibration curve compared to an adaptive calibration curve gave similar accuracies in the scanning step size range of 150-350 mu m (with an average overlap of the accuracy ranges of 92.94% for simulations and 42.83% for experiments), and a 350-fold improvement in computation time.

Background: Transrectal ultrasound-guided prostate biopsy (TRBx) or transperineal Bx (TPBx) are considered alternative approaches for the diagnosis of prostate cancer (PCa). However, urinary tract infection (UTI) or other complications could be more frequent in the TRBx approach. Objective: To determine the complication rate following different antimicrobial prophylaxis (AMP; fosfomycin trometamol [FT] vs beta-lactame or fluorochinolones [FQ]) in patients undergoing TRBx or TPBx. Design, setting, and participants: The analyses were based on prospectivelycollected data of a cohort of patients who underwent TRBx or TPBx for elevated prostate-specific antigen (PSA; >= 4 ng/ml) or clinical suspicion of PCa, between September 2016 and March 2017. Patients received a single dose of 3 g oral FT (group A) or, alternatively, FQ or beta-lactame (group B). Intervention:TRBx versus TPBx. Outcome measurements and statistical analysis: Adjustment variables consisted of age, PSA, biopsy technique (TPBx vs TRBx), and antibiotic prophylaxis (FT vs beta-lactame or FQ) using 1:1 propensity-score matching. Overall, 526 patients were considered, of whom 258 received FT (group A) and the other 258 received beta-lactame or FQ (group B). Results and limitations: Overall complications occurred in 390 (75.58%) and major complications in 67/516 (12.98%). Lower prevalence of UTIs was detected in group A (34.1%) compared with that in group B (43.4%; p = 0.03), while similar rates of haematuria (54.7% vs 55.4%), haemospermia (39.5% vs 33.0%), and acute urinary retention (11.6% vs 9.3%) were detected in groups A and B. We found that group B (odds ratio [OR]: 1.54; p = 0.03), I grade haematuria (OR: 6.17; p < 0.01), and II grade haematuria (OR: 5.13; p < 0.01) were significantly associated with increased risk of UTIs. Conclusions: AMP with fluoroquinoles or beta-lactam antibiotics increased the rate of UTIs, when compared with FT, in patients undergoing TRBx or TPBx. The appearance of haematuria or haemospermia is associated with UTIs, suggesting the possibility of tailoring the strategy for prophylaxis in this category of patients. Patient summary: In this study, comparing complications after transrectal ultrasound-guided prostate biopsy versus transperineal biopsy, prophylaxis with fluoroquinoles or beta-lactam antibiotics increased the rate of urinary tract infections when compared with fosfomycin trometamol, regardless of the type of biopsy approach. Background: The high prevalence of physician burnout, particularly in emergency medicine, has garnered national attention in recent years. Objective means of measuring stress while at work can facilitate research into stress reduction interventions, and wearable photoplethysmography (PPG) technology has been proposed as a potential solution. However, the use of low-burden wearable biosensors to study training and clinical practice among emergency physicians (EP) remains untested. Objective: This pilot study aimed to (1) determine the feasibility of recording on-shift photoplethysmographic data from EP, (2) assess the quality of these data, and (3) calculate standard pulse rate variability (PRV) metrics from the acquired dataset and examine patterns in these variables over the course of an academic year. Methods: A total of 21 EP wore PPG biosensors on their wrists during clinical work in the emergency department during a 9-hour shift. Recordings were collected during the first quarter of the academic year, then again during the fourth quarter of the same year for comparison. The overall rate of usable data collection per time was computed. Standard pulse rate (PR) and PRV metrics from these two time points were calculated and entered into Student t tests. Results: More than 400 hours of data were entered into these analyses. Interpretable data were captured during 8.54% of the total recording time overall. In the fourth quarter of the academic year compared with the first quarter, there was no significant difference in median PR (75.8 vs 76.8; P=.57), mean R-R interval (0.81 vs 0.80; P=.32), SD of R-R interval (0.11 vs 0.11; P=.93), root mean square of successive difference of R-R interval (0.81 vs 0.80; P=.96), low-frequency power (3.5×103 vs 3.4×103; P=.79), high-frequency power (8.5×103 vs 8.3×103; P=.91), or low-frequency to high-frequency ratio (0.42 vs 0.41; P=.43), respectively. Power estimates for each of these tests exceeded .90. A secondary analysis of the resident-only subgroup similarly showed no significant differences over time, despite power estimates greater than .80. Conclusions: Although the use of PPG biosensors to record real-time physiological data from EP while providing clinical care seems operationally feasible, this study fails to support the notion that such an approach can efficiently provide reliable estimates of metrics of interest. No significant differences in PR or PRV metrics were found at the end of the year compared with the beginning. Although these methods may offer useful applications to other domains, it may currently have limited utility in the contexts of physician training and wellness.

Monomeric zinc ferrocene carboxylate [Zn(FcCOO)(3,5-dmp)(2)Cl] (1) has been synthesized from a reaction between the zinc chloride, sodium salt of ferrocene carboxylic acid (FcCOONa) and 3,5-dimethylpyrazole (3,5-dmp) at room temperature. Compound 1 was further characterized by elemental analysis, H-1 NMR, FT-IR, absorption, emission spectroscopic techniques, and molecular structure was confirmed by single-crystal X-ray diffraction studies. The molecular structure of compound 1 crystallized in triclinic system with P (1) over bar space group and the central Zn2+ ion is in a distorted tetrahedral geometry with -FeN2OCl coordination environment. Compound 1 was further stabilized with the aid of inter and intramolecular hydrogen bonding and pi-pi interaction between cyclopentadienyl ring of ferrocene and aromatic heterocyclic five membered 3,5-dimethylpyrazole. Compound 1 exhibits broad emission band, indicating ligandto-metal charger transfer (LMCT) or metal-to-ligand charger transfer (MLCT) nature. The electrochemical property of compound 1 revealed that one-electron anodic and cathodic peaks corresponding to the redox responsible of ferrocene/ferrocenium (FeII <-> Fe-III) moiety. Among the different strains of bacteria, the antibacterial activity of compound 1 showed comparatively a significant activity against gram negative bacterium Proteus vulgaris. (C) 2020 Elsevier B.V. Background: The Islamic Republic of Iran is at high risk of natural disasters. Households are the basic unit of society and they need to be prepared for disasters. Aims: This study assessed disaster preparedness among Iranian households in 2015. Methods: Using multistage cluster sampling, 2968 households in the catchment area of 53 public health departments in all 30 provinces of the Islamic Republic of Iran were surveyed on the disaster preparedness measures they had taken in the past year. The data collection questionnaire included 15 disaster preparedness measures. The mean preparedness score was calculated and its association with residence (urban versus rural), house ownership (owner versus renter) and education level of the head of the household (illiterate, elementary school, middle school, high school, university) was evaluated. Results: The mean score for household disaster preparedness was 9.3 out of 100 (95% confidence interval: 8.3-10.3). Multivariate analysis showed higher preparedness among rural than urban households (P = 0.02). Higher education level was positively associated with a higher preparedness score (P < 0.001), as was house ownership (P < 0.001). Conclusion: The level of household disaster preparedness in the Islamic Republic of Iran in 2015 was low. In line with the first priority for action of the Sendai Framework for Disaster Risk Reduction, the current study provides an overall picture of household disaster preparedness in the Islamic Republic of Iran. This estimation can be used as a baseline value for policy-making, planning, and evaluation of public awareness of disasters in the country. Recommanded Product: 83-10-3. This study examined the potential to use a nitrification inhibitor (NI) assay system to simultaneously estimate the rates of net N mineralization and denitrification, and applied the NI assay to assess the effect of water content on net N mineralization and denitrification rates in two soils with contrasting soil texture. The compound 3,5-dimethylpyrazole (DMP) applied at a rate of 200 mg kg(-1) was found to provide essentially complete inhibition of nitrification over the duration of the soil incubation for two soils with contrasting soil texture (clay loam vs. sandy loam) and over a range of soil water contents (35%, 55%, and 85% water-filled pore space). This allowed net N mineralization to be estimated as the accumulation of soil ammonium (NHthorn 4) and of denitrification as the disappearance of added nitrate (NO3-). Addition of DMP resulted in a small increase in soil respiration rate but did not appear to influence the rate of net soil N mineralization. The NI assay provides a practical means to quantify the rates of net N mineralization and denitrification simultaneously over a wide range of soil water contents. The assay can be readily scaled up to routinely test multiple soils in an efficient manner, has limited material costs, and is also relatively simple to perform.
